Access our online application here. Additional instructions about how to successfully submit the application is provided within the online system. An application fee of $50 is required for all applications submitted.

    Fee Waivers
Application fee waivers are available to those who within the last three (3) years have:

    • Participated in a MCW high school and/or undergraduate research experience (i.e. DSHREP, PURE, SPUR, StEP-UP, etc.),
    • Received, or can provide proof of eligibility for, a GRE Fee Reduction Certificate
    • Attended a MCW Graduate School fair,
    • Participated in a local, state or national program that supports underrepresented student populations (i.e. Louis Stokes Alliance, McNair Scholars, etc.),

Eligibility can be indicated within the online application. The need for supporting documentation may be required to verify fee waiver eligibility.

    Transcripts
An official or unofficial transcript from any current or previously attended college/university is required as part of your application submission. Applicants are encouraged to arrange for their transcript(s) to be received by the MCW Graduate School at the earliest possible opportunity. Applicants can arrange for transcripts to be sent from the current or previous institution in the mail to the following address...

    MCW Graduate School
    8701 W Watertown Plank Road
    Milwaukee, WI 53226

...or via email to [email protected] through one of the following approved third-party vendors:

    • E-Scrip Safe
    • Parchment
    • National Student Clearinghouse

Official transcripts can be submitted directly by the applicant in the mail or in-person so long as they are in their original, sealed envelope.

Unofficial transcripts can be sent in the mail or via email attachment to [email protected]. NOTE: while we will accept unofficial transcripts (i.e. records not sent directly to us by the college/university or via an approved third-party vendor), no applicant will receive an offer of admission until an official record has been received. As a result, we encourage all applicants to consider sending us official records from the onset of their application process.

    Grade Point Average (GPA)
While we do not have a minimum requirement, a GPA at or above 3.0 (on a 4.0 scale) is generally recommended for an applicant to receive competitive consideration. Those with a GPA less than a 3.0 may still be considered for admission, since we consider aspects beyond academic performance such as current or previous professional and/or extracurricular activities, the depth of experiences related to the program that an applicant is applying to, and other factors.

    Test Score
As of 2019 admissions cycle, the MCW Graduate School no longer requires the submission of a test score. However, some of our admitting programs elected to preserve the requirement; these programs are listed below along with the test type they most prefer.

    • Biophysics (PhD) – GRE
    • Biostatistics (PhD) – GRE
    • Medical Physiology – MCAT

Any test offered through Educational Testing Services (ETS), such as the GRE, can be sent directly to MCW using institution code 1519. Applicants submitting MCAT scores can send their report via email as an attachment to [email protected] and must include the verification code and account number. GRE and MCAT scores will be accepted as far back as five (5) years from the date of the test. GRE scores in the 50th percentile or higher in all sections of the test are recommended for competitive consideration. Applicants who have earned a terminal degree may request an exemption to the test score requirement by submitting a written request via email to [email protected]. Applicants are encouraged to explore whether they may be eligible to receive a GRE Fee Reduction Certificate if they are required to submit a test score.

    English Language Proficiency Tests
The submission of an English language proficiency test score is generally required for international applicants who are not currently enrolled in, or have not previously graduated from, a U.S.-based (non-online) undergraduate or graduate degree program. As of the 2022 admissions cycle (staring July 1, 2021), we accept the following test types:

    • Duolingo (recommended score of 105 or higher)
    • IELTS (recommended score of 6.0 or higher)
    • TOEFL (recommended score of 100 or higher)

We will accept scores from any of these services as far back as two (2) years. An additional exception to the English language proficiency test may be approved for applicants who are currently enrolled in or previously studied at foreign institutions whose primary language of instruction was English. In these cases, applicants will need to submit an exemption request in writing via email sent to [email protected] after they have submitted their online application.

    Letters of Recommendation
Applicants will identify and send notice to their recommenders through the online application. In turn, recommenders will receive an email with instructions on how to submit their letter. It is the applicant’s responsibility to ensure that recommenders submit their letters on or before the priority application date of the program(s) that they are applying to. Letters will only be received by the recommender and not by the applicant or any third-party entity. There are three ways that the recommender can submit their letter:

Recommendation letters must can be submitted electronically through our online application system (preferred), as an attachment via email to [email protected], or in the mail. Applicants should refrain from having family members and those that they receive healthcare services from submit letters, even if those individuals happen to also to be one of the types of recommenders that we will accept letters from.

There is no particular order in which the online application or their associated materials need to be submitted/received. Applicants are encouraged to ensure that all their application and required materials have been submitted prior to their program’s priority application deadline date.

Anyone who is currently residing outside of the United States or are studying in the United States on any visa type are considered an international student applicant.

    School Records / Transcripts
International Applicants must provide official/original copies of their transcripts in the native language of instruction as well as in English translation. Transcripts are also known as academic records or mark sheets. An official transcript must bear the original seal and/or stamps of the institution as well as signatures of institutional officials. If transcripts do not include proof of graduation (i.e. diploma and/or degree certification), official copies of these documents must be provided as well. All academic credentials are reviewed and verified at our discretion. It is incumbent upon the applicant to ensure that the documents submitted meet the requirements of being considered official.

    English Language Proficiency Tests
The submission of an English language proficiency test score is generally required for international applicants who are not currently enrolled in, or have not previously graduated from, a U.S.-based (non-online) undergraduate or graduate degree program. How to Apply

    Primary Application (AMCAS)

The primary application to the Medical College of Wisconsin must be initiated through the American Medical College Application Service (AMCAS).

MCW Medical School offers a variety of program types, which are listed on the AMCAS application.

    • Regular MD: This program is offered at all three of our campuses.
    • Early Decision: This program is offered at all three of our campuses.
    • Combined MD/PhD or Medical Scientist Training Program (MSTP): This program is only offered at MCW-Milwaukee. This dual-degree program is typically completed in eight years, and matriculates about six students per year. Learn more
    • Combined MD/MS or MD/MS in Clinical and Translational Science: This program is only offered at MCW-Milwaukee. This dual-degree program is intended to be completed within four years, and is designed for students who wish to pursue a medical career with a research focus.

Combined MD/MPH: This program is only offered at MCW-Milwaukee. Applicants to this dual-degree program will be eligible to apply after their first year of medical school at MCW has been completed.

    Secondary Application

Secondary applications will be sent to all eligible applicants upon MCW’s receipt of your verified AMCAS application. If you received an email with login credentials for the MCW Secondary Application, please complete the application. If you have not received that email, or a communication that you are ineligible to apply, please check your spam or junk mail folders and contact the Office of Admissions.

You will designate your campus preferences on the secondary application. There is only one secondary application and one application fee, regardless of campus preferences.

Please familiarize yourself with the mission and opportunities available at each campus location before submitting your application. Since MCW-Central Wisconsin and MCW-Green Bay have small class sizes of primarily Wisconsin residents, there are limited spaces available for non-residents.

    Required Forms

These forms are not required of all applicants. Please review the requirements of each form, and complete the form(s) that pertain to you. All forms are in PDF format. You will need the free Adobe Reader to view and print the forms.

    • Dean’s Certification: This form is required if you have indicated on the AMCAS application or on our Secondary Application that an institutional action has been taken against you. It is also required if you have previously matriculated at a medical school regardless of the reason for departure. Provide this form with your explanation to the appropriate office at the institution for verification. It will then be sent directly to us by your institution. If applicable to you, this form is required to complete your application.
    • Wisconsin Residency Form: If you are considered a Wisconsin resident for tuition purposes, complete and send this form to the Higher Educational Aids Board (HEAB) in Madison, WI. This form must be approved prior to matriculation to qualify for the Wisconsin Medical Student Tuition Assistance program, but it is not required to complete your secondary application. Please note: Private colleges do NOT participate in the tuition reciprocity program because they do not charge non-resident tuition rates. Therefore, the Minnesota/Wisconsin reciprocity agreement does not apply to MCW students.
    • Technical Standards: The Technical Standards for Admission, Retention, Promotion and Graduation consist of the minimum physical, cognitive and emotional requirements to provide reasonable assurance that students can complete the entire course of study and participate fully in all aspects of medical training. Current students review and sign the Technical Standards annually.
    • Credit Report: All admitted and waitlisted applicants are required to submit a copy of their credit report to be approved by the Financial Aid Office by the deadline date provided in their acceptance or waitlist letter. Credit reports can be requested on the Annual Credit Report website. If you have no credit history, this form is required. Additional documentation may be required by the Financial Aid Office on a case-by-case basis.
    • Certification of Finances: If you are not a US Citizen, you must submit the Certification of Finances form and documentation to the Financial Aid Office. You are encouraged to complete and submit this documentation early after being admitted or placed on the waitlist; this is not required to complete your application but needs to be submitted and approved well in advance of matriculation.
    • International applicants who have been accepted to the Medical Scientist Training Program (MSTP) are not required to provide a Certification of Finances form since the MSTP program provides students with financial support during the duration of the program. If, for any reason, the international student leaves the MSTP program, the student would then be required to provide a Certification of Finances (amount to be determined). If this documentation is not approved, the student will then be withdrawn from MCW.
    Deferred Admission

MCW Medical School expects admitted applicants to matriculate in the class for which they were interviewed. In limited instances, an admitted applicant may request a one-year deferment of matriculation for personal reasons, professional development reasons, or extenuating circumstances. Examples include significant personal or family health challenges, childbirth or adoption, meaningful professional growth opportunities (e.g., Fulbright scholarship), service in the military or Peace Corps, or unforeseen circumstance that would seriously impact the applicant's ability to succeed as a medical student.

The request must be in writing using the appropriate form, must describe the specific reason(s) for the request, and may be submitted only after all stipulations of acceptance have been met. Students may only request deferment for the campus to which they have been admitted. Decisions to grant deferred matriculation are made on a case-by-case basis, and the MCW Admissions Committee reserves the right to limit the number of deferred matriculants in any entering class. Decisions on deferred matriculation requests will be communicated in May of the matriculation year.

    Transfer Policy

The Medical College of Wisconsin only considers transfer applications from students demonstrating hardship into its third-year curriculum at MCW-Milwaukee from LCME-accredited schools. This is rare, as it is contingent on available space.

The transfer application to matriculate in July 2023 with the M3 class is now closed.

For the M3 class beginning in July 2024, it is not yet determined if there is limited space available. All completed transfer applications will be reviewed after December 1, 2023. Interviews will be offered to eligible and qualified applicants with final decisions sent by the end of February 2024. If accepted for transfer, you must plan to be available and on-campus by mid-June 2024. View the Medical School academic calendar for additional details.

To complete a transfer application for potential matriculation as an M3 in July 2024, please complete the MCW Medical School Transfer Application. Again, the deadline for an entirely complete application is December 1, 2023. A complete application includes additional required documents outlined in that link.
