Certificate in An Overview of Long-term Conditions and Clinical Management at Teesside University

You identify common long-term conditions (LTC) using the LTC framework and discuss the pathophysiology, common symptom presentation and current gold standard clinical management guidelines. You place the service user at the core. This module can be accessed as an individual award by registered health care practitioners working in any field.

Study the common long-term conditions that affect the respiratory, neurological cardiovascular, musculoskeletal and gastrointestinal systems. You also look at the clinical presentation of frailty and long covid and discuss how to implement advanced care planning and start difficult conversations, as well as discuss the clinical management of patients with a cancer diagnosis.

You undertake face-to-face seminars over ten-weeks and self-directed study to consolidate this learning. Seminars are delivered by a mixture of lecturers and visiting clinical experts, with in class discussions encouraged to enable peer-to-peer learning.

You are assessed on a 3,000-word case study which includes:

• undertaking an appropriate and technically correct discussion of a systematic clinical examination of the patient, identifying understanding of the system being examined to include accurate identification of normal/abnormal findings and possible underlying pathology
• demonstrating high level cognition by appraising and evaluating the significance of the patient history including physical examination findings with current evidence
• demonstrating critical thinking, diagnostic reasoning and complex clinical decision-making skills in the proactive management of long-term health conditions
• incorporating evidence-based decisions in complex situations and accepting accountability for the outcome and impact of those decisions, appraising the impact of biopsychosocial factors on the person and their family/carers
• demonstrating safe practice throughout the assessment.

Tutorial support is available through the module lead and through peer discussion within taught sessions.
